The facility will, as allowed, maintain social distancing at all opportunities with exception of care provision. Meeting spaces will be large enough for staff to sit separated. Floors will be marked so visitors know where to stand upon entry. Offices which are known to have visitors present have been adjusted to allow for more space. Where space cannot be provided, staff will wera face coverings

Staff have been directed to use the mail, phone and computers for main methods of communication to minimize the necessity for in-person meetings. The nursing homes do not have telework options at this time so we will not likely be able to accommodate high-risk personnel needs. At this time visitors are still not permitted in the buildings.

The nursing home only has 2 areas of entry and both require staff to assist with access. Health screenings and issuance of PPE are occurring at both entries and will continue under the specific direction of the New York State Department of Health

Direct care staff in the nursing home will maintain social distancing to every extent possible when not providing required hands on care.
Hands-on care will be provided only while wearing the necessary PPE
Administrative, managerial and non-direct care staff will maintained social distancing while meeting in groups, in large meeting spaces and will continue to wear face coverings when social distancing cannot be maintained.
Residents, when not in their rooms,will wear face coverings, as tolerated.
Offices that must be shared, due to lack of other space available, will wear face masks when they cannot be alone in the office.

Please provide in detail any physical space modifications that are proposed to achieve social distancing requirements.  These modifications can include, but not limited to rearranging offices, adding physical barriers, etc.

The facility would like to look into purchasing a thermal body scanner, similar to what is used in Little Valley County building, to screen all visitors (and staff) at the front entrance, for the unforeseen future. The acting of taking someones ear temperature put the person taking the temp in very close proximity to everyone entering the building and is high-risk for infection transmission

PPE supplies have been and will continue to be maintained in a secured storage area for issuance to all who enter the building in a controlled manner. They are currently procured through the facilities regular channels of purchase as well as being acquired through the counties EOC.

The facility has maintained an on-going infection control and cleaning program throughout the COVID pandemic and will continue to do so. Resident rooms, common areas and all work spaces are cleaned and disinfected daily with extra focus on high-touch areas. Staff are reminded that PPE that needs replacing should be brought to supervisors attention so it can be replaced.

Nursing home staff have been screened for health since March and have been COVID tested since May 18th. These screening will continue under the instruction of the DOH. Positive COVID results are communicated immediately to the employee and the health department to initiate contact tracing and resident isolation where warranted. Signage regarding PPE, sociial distancing and other preventative measures are placed throughout the building and communicated via emails, memos and an employee newsletter.

Front entrance at main reception desk or supervisors office for off-shift. Both are located away from resident care areas
Staff will have their temperature taken immediately upon reporting to work. Anyone found with a temperature greater than 100 degrees will receive a COVID swab at the facility and be sent home pending results (usually 48 -72 hours). They will be returned to work when safe to do so.
